The Server/CAL licensing exists (since SQL Server 2012) only for the Standard Edition. The Server/CAL licenses pay off if you need low access licenses and can pinpoint accessing users or devices. On the server-side, there is a license per operating system environment (virtual or physical) in which the SQL Server instance is running. The number of installed SQL Server instances does not matter. You can install and run multiple SQL Server Standard instances with a license within a Windows Server instance (equivalent to an operating system environment).
The CAL part of the Server/CAL license model works in the same way as Exchange, SharePoint, etc. For different requirements, there are User and Device CALs.
Access licenses are versioned but not server-bound. With a SQL Server 2012 CAL, you can access SQL Server 2012 or older. As a result, CALs need to be rebuilt for an update without active Software Assurance coverage.
